Data Controller Application Form

1Introduction

Article 11 of the Personal Data Protection Law No. 6698 (the "Law") grants natural persons whose personal data is processed the right to apply to the data controller in order to obtain information about the status of the data relating to them, to monitor the processing and to exercise their statutory rights.

In this context, natural persons holding the status of Data Subject may apply to the Data Controller, Aksu Piliç Tarım Sanayi İç ve Dış Tic. A.Ş. (the "Company") in accordance with the procedures and principles set out below, and thereby to exercise their right to learn whether their personal data is being processed, to request detailed information if it is, to learn the purpose of processing and whether the data is used in accordance with that purpose, and to exercise the other rights listed in the Law.

2Application Method

In order for your applications to be concluded quickly, effectively, securely and lawfully, you must contact us by choosing one of the methods set out below, determined pursuant to the provisions of the Communiqué on the Procedures and Principles of Application to the Data Controller.

3.1. Data Subject Identity and Contact Details

The fields below must be completed so that we can contact you and verify your identity.

3.2. Your Relationship with Our Company

The nature of your relationship with our Company is important in enabling us to determine which departments process your personal data and within which processes. In order for your application to be assessed promptly by the correct units, please tick the appropriate option and add the necessary details.

3.3. Subject of the Request

Please set out your request concerning your personal data below, in sufficient detail and clarity for the subject to be clearly understood by the Data Controller. Stating which article of the Law your request is based on and attaching any information and documents supporting your request will speed up the review process.

3.4. Method by Which the Response Will Be Sent to You

For collection by proxy, a notarised power of attorney must be presented.

3.5. Declaration and Signature

In line with the requests I have set out above, I kindly ask that my application to your Company be assessed pursuant to Article 13 of the Law and related legislation and that I be informed accordingly. I declare that the information and documents I have provided in this Application Form are correct and that I have been informed that the Company may request additional information in order to conclude my application.

4Conclusion of the Application

Pursuant to Article 13(2) of the Law, requests duly submitted to our Company will be concluded, depending on the nature of the request, as soon as possible and within a maximum of thirty days, and as a rule free of charge. However, if the process requires an additional cost, the fee set by the Personal Data Protection Board may be charged. Pursuant to Article 13 of the Law, our responses will be sent to you in writing or electronically according to the method you have chosen.

5Explanation and Notice

This Application Form has been drawn up in order to establish your relationship with our Company on a legal basis, to identify fully and correctly any personal data of yours processed by our Company, and to enable your application to be answered correctly and within the statutory period.

Our Company reserves the right to request additional information, documents or verification for the purposes of identity verification and authorisation checks, in order to eliminate the legal and criminal risks that may arise from unlawful and improper data sharing and, in particular, to ensure the security of your personal data. Where the information you provide in the Application Form is not correct and up to date, or where an unauthorised application is made, our Company accepts no liability for requests arising from such incorrect information or unauthorised applications or for any damages that may arise.
